5Essential Survey

5Essential Survey

The 5Essentials Survey is taken by teachers and students state-wide and usually requires no more than 30 minutes to complete. The survey may also be taken by parents, although their responses are not included on the School Report Card. The information collected through the survey is rigorously reviewed and analyzed to generate a 5Essentials Report for each school. The 5Essentials Report includes a breakdown of teacher and student responses and, most importantly, provides a comprehensive picture of the school environment based on the five Essential areas critical for school improvement in a meaningful context of similar and successful schools.

 This survey provides an opportunity for students and teachers to have a voice in improving their schools. Your school’s participation can help identify areas of strength and those in need of support, with fine-grained questions surfacing specific avenues for improvement. Equipped with this knowledge, school leadership—including teachers, support staff, principals, and district administrators—can be better positioned to target resources and efforts to drive school success.

Parents that are interested in taking the survey, can follow the link below to the survey site. Click on the button: Parent Survey – Start Here.  You will be taken to a login in screen in which you need to identify the county that the district is located in and the school your student attends.  If you have students attending more than one school in the district, you will need to select one of the schools (you can’t select both) and begin survey.  Because our school district is in four different counties we had to select only one and the county selected is Livingston.  When you start to type Tri-Point in for the school name, you will be given 3 options to select, the Elementary in Kempton, the Junior High in Piper City or the High School in Cullom.
